1. Candidate Details:

• Studying District: Enter the district where the candidate is currently studying 4th class (2023-24).
• Surname: Candidate's surname.
• Name: Candidate's name.
• Father's Name: Full name of the candidate's father.
• Mother's Name: Full name of the candidate's mother.
• Gender: Male/Female.
• Caste: Choose from SC, ST, BC-A, BC-B, BC-C, BC-D, OC, EBC.
• Sub Caste: Specify the sub-caste if applicable.
• Category: General/Minority.
• Fishermen Caste: Indicate YES/NO.
• Orphan Status: Indicate YES/NO. If yes, specify if semi-orphan.
• CAP (Children of Armed Personnel): Indicate YES/NO.
• Disability Status: Indicate YES/NO.

2. Personal Details:

• Date of Birth: Enter the candidate's date of birth.
• Telangana Resident: Indicate YES/NO.
• Type of School: Govt/Govt Aided/Private.
• Annual Parental Income: Specify urban (Rs. 2 Lakhs) or rural (Rs. 1.5 Lakhs).

3. Contact Information:

• Address: Candidate's communication address.
• District: District of residence.
• Mandal: Mandal of residence.
• Assembly Constituency: Constituency of residence.
• Pin Code: Pin code of the candidate's address.
• Parent's Mobile Number: Contact number of the parent/guardian.

4. Previous Education Details:

• Provide details of 1st to 4th class study, including the year, medium of instruction, school name, and whether it is government, government-aided, or private.

5. Preferences:

• Society Preferences: TSW, TTW, MJPTBC, TREI (Specify top 4 preferences).
• School Preferences: Specify up to 8 preferred schools.
