Filling out the Know Your Customer (KYC) form is a mandatory step to secure your LPG connection. This form ensures that your identity and address are verified for the LPG service provider. This guide will walk you through the process of completing the KYC form accurately.

Step 1: Download the KYC Form

First, download the KYC form from the Bharat Gas website here.

Step 2: Personal Details

Fill in the personal details section using black ink and BLOCK LETTERS:

• Photo: Paste your passport-sized photo in the designated area.
• Name: Enter your first name, middle name, and last name.
• Gas Consumer Number: Provide your gas consumer number if you are already using LPG from IOC/BPC/HPC.
• Date of Birth: Enter your date of birth in the format DD/MM/YYYY.
• Close Relative Details: Provide the names of your father/spouse and mother.

Step 3: Address for LPG Connection / Contact Information

Fill in your address and contact information:

• Proof of Address (POA) Category Code: Select the appropriate code from the list provided in the instructions.
• House/Flat Number: Enter your house or flat number.
• Floor Number: Provide the floor number if applicable.
• Housing Complex/Building: Mention the name of your housing complex or building.
• Landmark: Enter a nearby landmark.
• Street/Road Name: Provide the name of your street or road.
• City/Town/Village: Enter the name of your city, town, or village.
• Pin Code: Enter your area’s pin code.
• District and State: Provide your district and state.
• Phone Numbers: Provide your mobile and landline numbers.
• Email ID: Enter your email address.

Step 4: Other Relevant Details

Provide additional details:

• Proof of Identity (POI) Category Code: Select the appropriate code for your proof of identity from the list provided.
• Card Number: Enter the number of your selected POI document.
• Ration Card Details (if available): Provide the state of issue and ration card number if you have one.

Step 5: Declaration

Read and sign the declaration: "I hereby declare that the information provided by me above is true and correct to the best of my knowledge and belief. I also confirm that in the event of any information provided by me is found incorrect / is incomplete and also in the event of any violation of Government Regulation related to the supply and distribution of LPG, IOC/BPC/HPC will be within its right to discontinue supply of LPG cylinders to me, forfeit of security deposit and levy of penal charges as per the policy and guidelines and may initiate legal action applicable under provisions."

• Signature of Applicant: Sign the form.
• Date: Provide the date of submission.

Step 6: Distributor Verification

This section will be filled by the distributor:

• Verification: Distributor will verify the photocopies of the documents against their originals.
• Consumer Number: (If allotted)
• Signature of Distributor: Distributor’s signature and date.
• Name of Distributorship with SAP Code: Name and SAP code of the distributorship.

Step 7: Acknowledgement Slip

The distributor will provide an acknowledgment slip upon receiving your KYC form:

• Receipt Details: Confirm receipt of the KYC form and relevant POI and POA documents.
• Signature and Seal of Distributor: Distributor’s signature and seal.
