Process for Adding a Name to a Birth Certificate in Punjab

Required Documents for Online Application

The following documents must be scanned from the original and uploaded online:

1. Proof of Identity and Address of Applicant: As per the Punjab government notification no. E-64139/2GR2/423 dated 25-11-2021.
2. Self-declaration from Father/Mother/Guardian.
3. Original Birth Certificate of the Child.
4. School Certificate / Passport / Immunization Card / Driving License: Any document that includes the name and date of birth of the child.
5. Passport Size Photo of Beneficiary (Child): Required if the applicant is not the beneficiary.

Self-Declaration Form

Self-declaration from Father / Mother / Guardian

1. My son/daughter was born on ______________.
2. At the time of birth registration, no permanent name was given, thus it was registered without a name.
3. I have now decided the permanent name of my child as ______________ in Punjabi and ______________ in English, and it will not be changed in the future.
4. I request the registration of my child's name ______________ in Punjabi and ______________ in English in the birth registration record, and the birth certificate should be issued accordingly.
5. The above information provided by me is true and correct, and I understand that providing false information is punishable under law, and any benefits obtained will be forfeited.
6. I declare that I will keep the file related to the birth certificate safe until it is issued, and I will present it in court if required.
