   • Enter your name, father's name, mother's name, sex, date of birth, area of residence, caste category, and disability status.
2. Address Details:
   • Provide your postal address for correspondence and the name and address of the institution where you're studying in Class VIII.
3. Institution Details:
   • Indicate the type of institution (government, government-aided, or local body) and the area in which the school is located.
4. Exam Details:
   • Select the medium of examination, father's education, father's occupation, mother's education, mother's occupation, number of family members living in the house, number of brothers and sisters, candidate's position among siblings, and parental annual income.
5. Contact Information:
   • Fill in the telephone STD code, phone number, mobile number (mandatory), and email ID (mandatory).
6. Declaration:
   • Ensure the declaration is filled in and signed by the head of the institution or a gazetted officer.

• Use blue or black ballpoint pen and write in capital letters.
• Do not mark or write outside the boxes provided.
• Leave one box blank between two words.
