Applying for a new LPG connection requires submitting a declaration form, which confirms your eligibility and the absence of another LPG connection in your household. This guide provides a step-by-step explanation of how to complete and submit the declaration form for a new LPG connection.

Step 1: Download the Declaration Form

You can download the official declaration form for a new LPG connection from the Bharat Gas website here.

Step 2: Fill Out the Declaration Form

Complete the form with the following details:

Step 3: Confirm Your Eligibility

Ensure that you enclose a duly filled KYC form along with proof of address (POA) and proof of identity (POI).

Confirm that the LPG connection will be used at the above-mentioned address and for domestic cooking purposes only. Also, confirm that no other LPG installations will be positioned in the same kitchen.

Step 9: Second Cylinder Usage

Declare that any second cylinder issued will be used in the same kitchen with the original installation.

Step 10: Change of Address

Undertake to inform the distributor in writing in advance if there is a change of address.

Step 11: Awareness and Compliance

Acknowledge that the connection issued is based on the declaration and information submitted by you. It will be subject to de-duplication checks.

Step 12: Accuracy of Information

Declare that all information provided is true and accurate. Acknowledge that any false information may lead to withdrawal of gas supply, termination of connection, seizure of equipment, and forfeiture of the security deposit.
