  1. Name (in BLOCK Letters): Write your full name in block letters.
  2. Date of Birth: Enter your date of birth.
  3. Email ID: Provide a valid email address.
  4. Aadhar No. of Student: Enter your Aadhar number.
  5. Father's Name & Aadhar No.: Provide your father's name and his Aadhar number.
  6. Mother's Name & Aadhar No.: Provide your mother's name and her Aadhar number.
  7. Mobile Nos.: Write your father's and your mobile numbers.

Section 4: Academic Details

  1. % Marks of Qualifying Semester: Fill in your percentage marks for each qualifying semester (I/II, III/IV, V/VI, VII) and the aggregate percentage.

Section 5: Address

  1. Address: Provide your complete address including district, state, and pincode.

Section 6: Category

  1. Category (GEN/OBC/SC/ST): Tick the appropriate category.
  2. Minority (Yes/No): Indicate whether you belong to a minority community.

  1. Gender (Male/Female/Trans): Tick the appropriate gender.
