Step 1: Obtain the Application Form

Download the Worker Registration Form from the official website of the Department of Labor, Punjab, or collect it from a local Sewa Kendra.

Step 2: Fill in Personal Details

  1. Application Number: Leave this field blank (for official use only).
  2. Date of Application: Enter the date you are filling out the application.
  3. Name of Block & Tehsil: Provide the name of your block and tehsil.

Step 3: Details of Person Filling the Application Form

  1. Name: Enter your full name.
  2. Address: Provide your complete address.
  3. Relation with Beneficiary: Mention your relation with the beneficiary.

Step 4: Personal Data of Beneficiary

  1. Name: Enter the beneficiary’s full name.
  2. Gender: Select the appropriate option - Male, Female, or Transgender.
  3. Date of Birth: Enter the beneficiary’s date of birth.
  4. Age: If the date of birth is not known, provide the approximate age.
  5. Place of Birth: Enter the place of birth.
  6. Father’s Name: Enter the father’s name.
  7. Mother’s Name: Enter the mother’s name.
  8. Address: Provide the permanent address of the beneficiary.

  12. Aadhaar Number (UID): Enter the Aadhaar number.
  13. Aadhaar Enrollment Number: If Aadhaar is not issued, provide the Aadhaar enrollment number.

  1. Mode of Delivery of Service: Select the mode of delivery - Sewa Kendra or By Post.
  2. Application Processing Office: Enter the details of the office processing the application.
  3. District: Provide the district name.
  4. Name of Circle of the Officer: If unsure, leave it blank.
  5. Name of the Circle of the Inspector: If unsure, leave it blank.
  6. Area: If unsure, leave it blank.
  7. Type of Work: Specify the type of work.
  8. Name of Worker: Enter the worker's name.
  9. Name of Husband / Father: Enter the name of the husband or father.
  10. Date of Birth: Enter the date of birth (between 18-60 years).
  11. Qualification: Provide the educational qualification.
  12. Gender: Select the gender.

  1. H.No/Street/V.P.O/Mohalla: Provide the full address.
  2. District: Enter the district name.
  3. City / Sub Div.: Enter the city or subdivision name.
  4. Pin Code: Provide the pin code.
  5. Email Address: Enter a valid email address.
  6. Mobile Number: Provide a mobile number.
  7. Phone Number: Provide a phone number (if applicable).
  8. Employer Name: Enter the employer’s name.
  9. Employer Address: Provide the employer’s address.
  10. Are you Migrant?: If yes, fill in the details of the state, district, and city.

Step 7: Additional Information

  1. Member of any other Board: If yes, provide the details of the board and the registration number.
  2. Bank Account Details: Provide the bank details for transferring benefits.

  3. Family Member Details: Provide details of family members as per the definition of family.

Step 8: Required Documents

Attach the following documents:

  3. Bank Passbook: Copy of the bank passbook.
  4. Form 27 Certificate by Employer/Contractor: Regarding employment of the building worker.
  5. Form 29 Nomination Form: Nomination form as per the rules.
