Step 1: Obtain the Application Form
Download the Worker Registration Form from the official website of the Department of Labor, Punjab, or collect it from a local Sewa Kendra.
Step 2: Fill in Personal Details
- Application Number: Leave this field blank (for official use only).
- Date of Application: Enter the date you are filling out the application.
- Name of Block & Tehsil: Provide the name of your block and tehsil.
Step 3: Details of Person Filling the Application Form
- Name: Enter your full name.
- Address: Provide your complete address.
- Relation with Beneficiary: Mention your relation with the beneficiary.
Step 4: Personal Data of Beneficiary
- Name: Enter the beneficiary’s full name.
- Gender: Select the appropriate option - Male, Female, or Transgender.
- Date of Birth: Enter the beneficiary’s date of birth.
- Age: If the date of birth is not known, provide the approximate age.
- Place of Birth: Enter the place of birth.
- Father’s Name: Enter the father’s name.
- Mother’s Name: Enter the mother’s name.
- Address: Provide the permanent address of the beneficiary.

- District
- Marital Status: Select the appropriate marital status.
- Spouse Name: Provide the spouse's name (if applicable).
- Email ID: Enter a valid email address.
- Contact Phone Number: Provide a contact phone number.
- Voter ID Card Number: Enter the Voter ID card number.
- Aadhaar Number (UID): Enter the Aadhaar number.
- Aadhaar Enrollment Number: If Aadhaar is not issued, provide the Aadhaar enrollment number.
- Phone Number / Mobile Number: Provide a contact number.

- Mode of Delivery of Service: Select the mode of delivery - Sewa Kendra or By Post.
- Application Processing Office: Enter the details of the office processing the application.
- District: Provide the district name.
- Name of Circle of the Officer: If unsure, leave it blank.
- Name of the Circle of the Inspector: If unsure, leave it blank.
- Area: If unsure, leave it blank.
- Type of Work: Specify the type of work.
- Name of Worker: Enter the worker's name.
- Name of Husband / Father: Enter the name of the husband or father.
- Date of Birth: Enter the date of birth (between 18-60 years).
- Qualification: Provide the educational qualification.
- Gender: Select the gender.
Step 6: Address Details
- H.No/Street/V.P.O/Mohalla: Provide the full address.
- District: Enter the district name.
- City / Sub Div.: Enter the city or subdivision name.
- Pin Code: Provide the pin code.
- Email Address: Enter a valid email address.
- Mobile Number: Provide a mobile number.
- Phone Number: Provide a phone number (if applicable).
- Employer Name: Enter the employer’s name.
- Employer Address: Provide the employer’s address.
- Are you Migrant?: If yes, fill in the details of the state, district, and city.

- Member of any other Board: If yes, provide the details of the board and the registration number.
- Bank Account Details: Provide the bank details for transferring benefits.

Step 8: Required Documents

- Form 27 Certificate by Employer/Contractor: Regarding employment of the building worker.
- Form 29 Nomination Form: Nomination form as per the rules.
