Step 2: Fill in Personal Details
Details of Person Filling the Application Form
- Self Attested Photo of Beneficiary: Attach a recent self-attested passport-size photograph.
- Name: Enter the full name of the applicant.
- Address: Provide the complete address of the applicant.
- Relation with Beneficiary: Specify the relationship with the beneficiary if the applicant is filling the form on behalf of someone else.
Personal Data of Beneficiary
- Name:* Enter the full name of the beneficiary.
- Gender:* Check the appropriate box: □ Male □ Female.
- Date of Birth: Enter the date of birth.
- Age: Mention the age if the date of birth is not known.
- Place of Birth: Enter the place of birth.
- Father's Name:* Enter the father's full name.
- Mother's Name: Enter the mother's full name.
- Address:* Provide the current address of the beneficiary.
- PIN Code:* Enter the postal PIN code.
- District:* Enter the district name.
- Marital Status: Check the appropriate box: □ Married □ Unmarried □ Divorced □ Widowed.
- Spouse Name: Enter the spouse's name, if applicable.
- Email ID: Provide the email address.
- Contact Phone Number:* Provide the contact phone number.
- Voter ID Card Number: Enter the Voter ID card number.

- Property Detail: Specify the type of property: SCF/SCO/SSS/Booth/Ind. Site/House/Plot/Other.
- Plot Number/Property Number:* Enter the plot or property number.
- Sector/Location:* Enter the sector or location.
- City:* Enter the city.
Step 4: List of Required Documents
Attach the following documents to your application form. Ensure each document is ticked (√) in the relevant box to confirm its inclusion:
- Proof of Ownership: Allotment Letter/ Re-allotment letter/Letter of change of ownership.
- Submission of Building Plans: Duly stamped "Sanctioned" and signed by an Architect empaneled by the concerned authority.
- 2 copies if plot size is <300 Sq. Yds.
- 3 copies if plot size is >300 Sq. Yds.
- Certified Copy of GPA/Sub Attorney: If applicable.
- Security/Scrutiny Fee: In the form of a Bank Draft in favor of the Executive Officer of the concerned authority.
- Copy of Receipt of Payment Made Towards Labour Cess.
