Step 1: Collect Required Documents

Before you begin filling out the application form, make sure you have the following documents ready based on the item type you are adding:

For Seeds:

  1. Source of Seed.
  2. Application along with Original License.

For Fertilizers:

  1. Source / Form O of all items.
  2. Application along with Original License.
  3. Declaration of Responsible Person under Clause 24 of FCO, 1985 for New Items.

For Insecticides:

  1. Residence Proof.
  2. Application regarding Addition of Insecticide as per Principal Certificate.
  3. Original Principal Certificates (Optional).

Step 2: Fill Out Personal Details

Details of Person Filling the Application Form:

  • Self-Attested Photo of Beneficiary.
  • Name of the person filling the form.
  • Address of the person filling the form.
  • Relation with the beneficiary (if different from the person filling the form).

Personal Data of Beneficiary:

  • Name*.
  • Gender* (Male/Female).
  • Date of Birth.
  • Age (if Date of Birth is not known).
  • Place of Birth.
  • Father's Name*.
  • Mother's Name.
  • Address*.
  • PIN Code.
  • District*.
  • Marital Status.
  • Spouse Name.
  • Email ID.
  • Contact Phone Number*.
  • Voter ID Card Number.
  • Aadhaar Number (UID) or Aadhaar Enrollment Number (if Aadhaar is not issued).

Step 3: Provide Service Details

Application Details:

  • Mode of Delivery of Service* (Sewa Kendra/By Post).
  • Application Processing Office.

License Type:

  • Select the type of license:
    • Fertilizer.
    • Insecticides.
    • Seeds.

License Number: Provide your current license number.

Is Existing in EDA: Indicate if the item is existing in EDA (Yes/No).
