How to Fill Out the ESIC Para Medical Admissions Application Form:

Personal Details:

• Enter the student's name, father's name, and mother's name as per the SSLC/10th marks card.
• Provide the date of birth in DD/MM/YYYY format and specify the gender.
• Indicate the religion, mother tongue, nationality, and category (UR/EWS/OBC/SC/ST/PWD).
• Enter the contact number, blood group, Aadhar number, and email ID.
